NFC Organic Certification Application Forms
The NFC Organic Certified Program conforms to the standards of the USDA National Organic Program. In addition to specific product application forms applicable to your company's products (see descriptions below), all new applicants for organic certification should download the following:
NFC Procedural Outline for Organic Certification
- Please Note: If you sell less than $5,000.00 of organic product per year, you are exempt from certifying with a USDA certification agent. (however, as an organic producer you still must adhere to the regulations of the USDA NOP)
- Restaurants, retailers, companies that have organic product pass through their facilities (and products are not opened while in facility), are exempt from certification.
- After application is completed, send it to our offices at 119A South Main Street, Spring Valley, NY 10977. Application must be accompanied with deposit and all necessary forms described below. (Incomplete application forms will delay the certification process.)
Organic Processing and Handling Application
This application is for manufacturers of and handlers of organic product. You should also submit an organic system plan with this application. (This is a written plan that sketches out the plans you have for your organic operation for the next year. See section 205.201 of the NOP standards for a detailed description what you should include). Attach the Organic Product Profile sheet included at the end of this file for each product requested for certification, a current schematic product flow chart, an organic handling and facility map for each facility which will handle organic products. Use additional sheets if necessary.

Organic Processing Re-certification Short Form Application
Current NFC certified entities use this form for continuing certification each new year (at the anniversary date of original certification).
NFC Organic Farm Certification Application
This form is used if you are requesting organic certification of your agricultural production. You must submit an Organic farm system plan with this application. This is a written plan that sketches out the various plans you have for your organic operation for the next year. See section 205.201 of the NOP standards for a detailed description what you should include.
Use additional sheets if necessary. Attachments required with questionnaire: Directions to your farm operation; farm map showing all fields, pasture areas and buildings and Pasture History Sheets.

Organic Livestock Plan Questionnaire: Slaughter/Dairy
This form is if if you are requesting organic certification of livestock slaughter stock (beef, sheep, etc.) and dairy stock.
You must submit an Organic system plan with this application. This is a written plan that sketches out the various plans you have for your organic operation for the next year See section 205.201 of the NOP standards for a detailed description what you should include.
A separate organic Farm Plan Questionnaire must be filled out if you are growing your own feed or other organic crops for sale. Use additional sheets if necessary. Attachments required with questionnaire: Directions to your farm/ranch operation; farm map showing all fields, pasture areas and buildings; water test of primary source of drinking water; and Pasture History Sheets.

For current NFC certified entities requesting continued certification on anniversary date of original certification.
Organic Poultry Plan Questionnaire
These forms are for organic certification or continuing certification of poultry for slaughter or egg production. A separate organic Farm Plan Questionnaire must be filled out if you are growing your own feed or other organic crops for sale. Use additional sheets if necessary. Attachments required with questionnaire: You must submit an Organic system plan with this application. (This is a written plan that sketches out the various plans you have for your organic operation for the next year See section 205.201 of the NOP standards for a detailed description what you should include).
Directions to your farm/ranch operation; farm map showing all poultry houses and outdoor access areas; Outdoor Access/Pasture History Sheet. For current NFC certified operations, please select the 'No Changes' option where applicable, and move on to the next section. Complete all sections where any changes have been made in the operation. Attach Poultry Outdoor Access History sheets for new areas or changes in pastures, and applicable test results.

Restaurant Certification Forms
Restaurants are exempt from USDA certification. NFC Restaurant Certification follow the standards developed by NFC.
